Last year, we had a “tripledemic” of COVID, Flu and RSV infections that all challenged the healthcare system at once. This year, it appears that it is a “syndemic” per Dr. Raj Rajnarayanan, as there are more than three pathogens that will impact our healthcare systems this winter. Specifically, we are seeing COVID, Flu, RSV and an atypical bacterial pneumonia in children caused by Mycoplasma Pneumoniae. An epidemic of Mycoplasma Pneumoniae lung infections are being seen in children in China, France, the Netherlands, Denmark, Ireland and Warren County, Ohio.
We know that prior COVID infections can increase the risk of RSV infections in young children, so it may be that past or present COVID infections are increasing the spread of Mycoplasma Pneumoniae in many countries now. Mycoplasma Pneumoniae pneumonia (MPP) is also known as “walking pneumonia”. According to Dr. Annie Sparrow, associate professor at the Icahn School of Medicine at Mount Sinai, the Mycoplasma Pneumoniae bacteria in China may be resistant to azithromycin, the only antibiotic approved for children under 8. I have not seen reports of antibiotic-resistant MPP from other sources and I cannot confirm that this is true.

Per Dr. Celine Gounder, Warren County Ohio is reporting an outbreak of pediatric pneumonia cases. This is similar to the "white lung syndrome" (aka ARDS) that is being seen in Europe and China. She notes that the increase in pneumonia in children is not due to a new pathogen, but rather, it is due to a combination of typical winter viral infections (RSV, Flu, COVID, adenovirus, rhinovirus) and bacterial Mycoplasma Pneumoniae. She tweeted “If this were a novel pathogen, adults wouldn't have immunity & would also be getting sick. That's NOT what we're seeing. Kids are getting sick because they haven't been as exposed to these respiratory illnesses as adults & aren't immune.” She recommends that parents “Take action to prevent RSV, COVID & flu – the 3 big viral respiratory illnesses.” by vaccinating children against these viruses.
RSV season is in full-swing, per Dr. Mandy Cohen of the CDC, and RSV cases are expected to peak soon. Flu season has also started and Dr. Cohen expects to see a lot more flu cases over the course of December and January. You can find a Flu vaccine appointment (and a COVID vaccine appointment) at vaccines.gov. COVID is causing the most hospitalizations of all of the respiratory infections, so it is especially important to be vaccinated with the updated XBB.1.5 vaccine. Only 16% of American adults have gotten the new COVID vaccine so far, while 37.5% have gotten a flu shot. Prior Omicron infection does not protect well against the newer variants, but COVID vaccination does.
Hospitalization for the Flu and for RSV are also increasing and outpatient visits for “respiratory illnesses” are abnormally high like last year. A new study shows that getting a flu shot can help also protect your heart. Flu vaccination was associated with a 26% decreased risk of heart attack and a 33% reduction in cardiovascular deaths.
The healthcare infrastructure in the U.S., and in many places around the world, has been severely stressed by high hospitalizations and severe healthcare worker burnout, so we need to prepare for this winter's respiratory illnesses. Vaccination against flu and COVID, improving air quality with air filtration, masking indoors with an N95 or KN95 mask, staying home when sick, testing for COVID every 48 hours if exposed or if you have symptoms, are all important ways to reduce the spread of respiratory viruses this season.
The CDC started a new and improved wastewater virus reporting system this week. COVID wastewater levels in the U.S. are “HIGH” per the CDC, especially in HHS regions in the midwest. COVID emergency room visits and hospitalizations are especially high in Region 5 (Illinois, Indiana, Michigan, Minnesota, Ohio, and Wisconsin), Region 7 (Iowa, Kansas, Missouri, and Nebraska), and Region 8 (Colorado, Montana, North Dakota, South Dakota, Utah, and Wyoming).
COVID hospitalizations are up 10% week over week while COVID deaths have plateaued. Although HV.1 accounts for 37% of COVID variants in the U.S. now, JN.1 is quickly overtaking all variants due to its superior growth ability. Fortunately, the updated XBB.1.5 COVID vaccine boosts neutralizing antibodies against both HV.1 and JN.1.
The JN.1 variant is spreading rapidly around the world and is causing increased COVID hospitalizations. JN.1 is not expected to cause more severe disease than other Omicron variants according to the WHO, but the upcoming large surge in cases could really stress our hospitals and healthcare systems. Consider getting the updated vaccine now in preparation for a new JN.1 wave that will be very high in mid to late December. Per JP Weiland, another COVID wave has started and the combination of the FLip variants + HV.1 wave happening as the JN.1 variant takes off, will cause anywhere from 1 million to 1.5 million new COVID cases per day in the U.S. by late December. Yikes!
The Federal government is giving 4 more free COVID tests per household at COVIDTests.gov. The non-profit Project N95 who sold reliable masks and PPE since early in the pandemic will be closing on December 15.
A new article in PNAS warns of a “second pandemic of mood and anxiety disorders, including major depression, anxiety and PTSD." The authors reviewed scientific findings on the neurobiology of stress and reviewed the roles of scientific discovery in both the short term and long term. Dr. Blitshteyn responded with an interesting viewpoint, “We can't talk about a second pandemic of mental health disorders without talking about the first pandemic: COVID-19 has significant neuropsychiatric manifestations, for which we have NO EFFECTIVE treatment. No amount of psychotherapy or positive self-talk is going to fix neuroinflammation.”
Vaccines
As mentioned previously, the Ho lab reported that the new 2023 updated XBB.1.5 COVID vaccine boosts neutralizing antibodies against XBB.1.5, EG.5.1, and key emergent variants like HV.1, HK.3, and JN.1.
There have been several studies linking COVID infection with premature birth. A new population level study shows that maternal COVID vaccination reverses this trend, thus preventing premature births. Preterm birth can have lifelong consequences for the health and socioeconomic well being of the child, so this is welcome news.
Only 16% of American adults are vaccinated with the updated XBB1.5 COVID vaccine this year, despite it being protective against the latest COVID variants. Dr. Katelyn Jetelina reviewed vaccine disparities and the increasing gap in COVID vaccination for Black and Latino people as compared to Whites. “Those who are more likely to lack access to healthcare are even more likely to be infected and get seriously ill once again. The commercialization of vaccines has only amplified gaps in access and uptake.” A new KFF poll shows that most people have not gotten the latest COVID vaccine and half are not taking precautions this holiday season. Dr. Jerome Adams, former U.S. Surgeon General tweeted about the KFF poll saying “Many White adults remain unvaccinated due to low concern about COVID, while Black and Hispanic adults cite barriers like limited time off work.” He also commented on the findings that Black and Hispanic people are more likely to take precautions against the spread of COVID including the use of masks and COVID testing before family gatherings.
Long COVID
A study of almost 600,000 people in Sweden shows that COVID vaccination protects against getting Long COVID- with 1 dose showing 21%, 2 doses 59%, 3 doses 73% protection. Dr. Akiko Iwasaki briefly reviewed the latest long COVID research on symptoms, testing and treatments for the AMA last week. A longitudinal study shows that there are 3 levels of clinical severity in Long COVID and that half of patients had fluctuations between severity types.
A press release from the Radiological Society of North America (RSNA) shows that by using Diffusion Microstructure Imaging (DMI), a novel MRI technique, investigators found that patients with Long COVID had specific patterns of microstructural brain changes that were different from people who recovered from a COVID infection without Long COVID.
Another presentation from the RSNA showed that a CT-guided Stellate Ganglion Block (SGB) may repair distorted sense of smell in COVID survivors.
In 2022, Nick Beauchamp of Northeastern University reported that when there was an increase in negative Amazon reviews for Yankee Candles saying “This Candle Has No Smell” correlated with rising COVID cases. This week, Salvatore Mattera shared a Reddit thread on r/Millennials starting with "Does anyone else feel like Covid broke their brain / not themselves?" In the thread, millennials discuss how since they had a COVID infection, they cannot think as clearly or they have memory loss, or they sweat a lot more or have loss of balance (dysautonomia), or they have loss of taste/smell. It makes me wonder how many more people have Long COVID and do not realize what it is.
Dysautonomia International posted on Twitter about a poster (Poster #68, page 25-26) from the 34th International Symposium on the Autonomic Nervous System showing that in two animal models of SARS-CoV-2 infection, there was structural damage to the autonomic ganglia even before respiratory symptoms started. They recommend studying autonomic ganglia pathology in humans.
Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a disabling post-infectious disease that is seen in many Long COVID patients. Although the following articles are not about Long COVID per se, they are about ME/CFS in general.
Ron Davis’ lab from Stanford showed that 47% of 19 ME/CFS patients and 5% of 19 healthy controls had plasma antibodies that could digest MBP (myelin). Glatiramer acetate (copaxone) inhibited this. They postulated that white matter demyelination in ME/CFS may be causing nerve pain and muscle weakness symptoms and that ME/CFS patients with general malaise and fatigue symptoms may have poor energy levels due to other issues including oxidative stress and mitochondrial dysfunction.
Regarding mitochondrial dysfunction and ME/CFS, Simmaron Research is recruiting 100 ME/CFS patients for a pilot study on Rapamycin, a mitochondrial enhancer, to treat ME/CFS. Their hypothesis is that “ME/CFS patients with elevated serum levels of pATG-13 and related autophagy disruption markers who are given low dose rapamycin will have these proteins (ATG-13) normalize and this may trend with a reduction in ME/CFS symptoms including PEM (post-exertional malaise) flairs.”
A review and meta-analysis this week on neuroinflammation in ME/CFS patients shows that the frontal cortex is most affected, and that the insula, thalamus and the limbic system all showed significant hypoactivity. They postulated that cortical-limbic disconnection could lead to abnormalities of metabolites and brain waves, which may contribute to the main symptoms of ME/CFS.
In non-COVID news, a new study shows that immediate skin-to-skin contact (kangaroo care) is very beneficial for premature babies and leads to increased infant positive affect, communication and social skills, and higher quality mother-infant interactions at 4 months of age. Children exposed to CT scans have a higher risk of blood cancers with a “clear dose–response between cumulative radiation dose to the active bone marrow (ABM) and the risk of blood cancers such as leukemias and lymphomas.”
CAR T-therapies for cancer may cause a new T cell cancer, but the FDA says that the benefits still outweigh the risks. New CPAP machines sent to people by Philips to replace the old ones with loose foam are now reported to be a possible fire hazard. There are reports to the FDA “of people facing thermal issues such as fire, smoke, burns, and other signs of overheating while using Philips’ DreamStation 2 CPAP machines.” Adults with Type I diabetes who used a hybrid closed-loop system were found to have fewer severe hypoglycemia events and DKA over 1 year. Mailing patients a human papillomavirus (HPV) self-sampling kit was successful in increasing cervical cancer screening.

THIS IS NOT DUE TO A NOVEL PATHOGEN CAUSING A NEW PANDEMIC.
3/ This seems to be a similar phenomenon to the pediatric pneumonia outbreak in China... but that is NOT to say that they are connected.

People with long COVID exhibit patterns of changes in the brain that are different from fully recovered COVID-19 patients.
They used DMI and compared MRI brain scans of three groups: 89 patients with long COVID, 38 patients that had contracted COVID-19 but did not report any subjective long-term symptoms, and 46 healthy controls with no history of COVID-19.

All eight patients were subsequently diagnosed with MCAS and all patients experienced significant improvements regarding neuropsychiatric and multisystemic symptoms after mast-cell-directed therapy.
In neuropsychiatric patients who have systemic symptoms and syndromes, it is important to consider the presence of an underlying or comorbid MCAS.

Some patients with ME/CFS have catalytic autoantibodies in their plasma that cause the breakdown of myelin basic protein (MBP).
Within the ME/CFS samples tested for this particular set, 47% were found to digest MBP, while in the healthy control group, 5% of the samples also showed digestion of MBP.
ME/CFS patients whose plasma Ab samples did not breakdown MBP may have symptoms unrelated to demyelination. They may be experiencing symptoms more related to general malaise and fatigue as might be caused by poor energy levels due to oxidative stress and mitochondrial dysfunction.
Glatiramer acetate (copaxone), an FDA approved immunomodulator to treat multiple sclerosis, inhibits ME/CFS antibody digestion of MBP.
Aprotinin, which is a specific serine protease inhibitor, specifically prevents breakdown of MBP. Catalytic antibodies may have serine protease-like activity.
"Because COVID-19 has been described as a trigger for ME/CFS, demyelination could play a bigger role in patient symptoms for those recently diagnosed with ME/CFS. Therefore, by studying proteolytic antibodies in ME/CFS, their target substrates, and inhibitors, a new mechanism of action could lead to better treatment and a possible cure for the disease."

This study looked at almost 1 million children and young adults who underwent lower dose CT examinations before age 22 years.
CT scan radiation was associated with an increased risk of blood cancers with a dose-response relationship.
